Register with NMPG patient account number The NMPG Patient Account Number appears on all NMPG patient statements and receipts.

Within one business day of completing this online registration form, you will receive an email from NMPG with a link to complete registration and begin using your new account.

Note: If you don’t have an old statement or receipt available, please ask for your Patient Account number at your next office visit.
